You certify that you are over 18 years of age and acknowledge that your participation in a gait analysis carries certain inherent risks. By participating, you voluntarily assume all risks associated with a gait analysis.

You certify that you are physically fit and able to participate in a gait analysis. You understand that it is your responsibility to consult with a healthcare professional prior to engaging in any strenuous physical activity.

You hereby release Whirlwind Sports Limited, its officers, directors, members, volunteers, and affiliates from any and all liability, claims, demands, actions, or causes of action arising out of or related to any loss, damage, or injury, including death, that may be sustained by you or caused to any property belonging to you while participating in Whirlwind Sports Limited activities.

You understand that participation in a Whirlwind Sports Limited gait analysis is entirely voluntary, and assume all risks and responsibilities associated with your participation. You agree to comply with all instructions, guidelines, and rules set forth by Whirlwind Sports Limited, its organisers, and appointed officials.

You also agree to treat all staff, participants, organisers, volunteers, and members of the public with respect. Disruptive, unsafe, or disrespectful behaviour, including harassment or discrimination of any kind, will not be tolerated and may result in exclusion from future activities.

You understand that Whirlwind Sports Limited and its affiliates may take photos and/or videos with you in them during a gait analysis, and you confirm that you grant your permission for these to be used for marketing purposes.
